A Medical Document is the authorization that allows you to register as a medical cannabis patient in Canada and purchase from a licensed medical platform like Mendo.

What a Medical Document is (and isn’t)

  • It’s similar to a prescription, but it’s not the same thing.

  • It’s a form completed by a physician or nurse practitioner that authorizes the use of medical cannabis and sets your daily amount (grams per day).

  • It’s used to support your registration and enable ordering through a medical cannabis provider.

Do I need a Medical Document to order from Mendo?

Yes. A valid Medical Document is required to:

  • Complete your medical registration

  • Activate your account for ordering

  • Ensure your profile includes an authorized daily amount

How to get a Medical Document through Mendo

Mendo can help you obtain a Medical Document at no cost through an online medical clinic with virtual appointments.

Step 1: Create a Mendo Account

If you have not already done so, register for an account with Mendo to set up your profile. You can use the button below to register.

A Note For Canadian Veterans:

When creating your account, make sure to select "I am a veteran" and provide your K-Number. This ensures you're account is ready for Direct Billing once you're approved!


Step 2: Book & Attend Your Consultation

After you've created your Mendo account, it's time to book your online consultation with our clinic. Mendo offers this consultation free of charge. Use the link below to book your online consultation.

You'll need the following information to book your consultation:

  1. First & Last Name

  2. Date of Birth

  3. Phone Number

  4. Email Address

  5. Health Card Number & Version Code

What happens during the consultation?

During your appointment, the clinician will review your information, medical history, and symptoms. This is also the time for you to ask any questions you may have about specific types of products and their potential effects. They can also provide general education on formats and how to read product information. The clinician will complete your Medical Document including your recommended grams per day.


Step 3: Wait For Approval

After your consultation, the clinician will send your Medical Document directly to Mendo. Once it’s received and reviewed, we’ll notify you via email and provide a copy for your records. When your account is approved, you can begin placing orders through the Mendo platform.


Using an existing Medical Document

Already have a valid Medical Document from another LP?

If you already have a Medical Document with a different licensed producer (LP), you can request a transfer to Mendo. If you have prescriptions or allotments with multiple providers, you can consolidate them under Mendo by contacting each provider to request transfers. For a streamlined process, you may also ask your healthcare provider to issue a new consolidated prescription directly to Mendo. Once the transfer or consolidation is completed, ensure any old prescriptions with other providers are deregistered to avoid conflicts.

  1. Email your current LP and ask them to transfer your Medical Document to us.

  2. Provide them with our LP name and fax details:

  • Secure fax line (for receiving Medical Documents): 514-370-2496

Important note about document submission

For privacy and compliance reasons, we can’t accept copies of Medical Documents directly from patients. Your Medical Document must be sent to us by a clinic, physician/nurse practitioner, or an LP (for transfers).

Caregivers assisting patients can also request medical document transfers on behalf of those they are supporting. Always confirm with providers that the secure fax number (514-370-2496) is used for document submissions to ensure privacy and compliance.
